Reissued Diplomas and Transcripts
Verification of Graduation by Phone
To verify high school graduation, please call 1-877-453-2721. To process the request, you must have the following information about the student:
- Name at the time of graduation
- Date of birth
- Year of graduation (and month if known)
- Name and location (school district and city) of high school
Copies of General Educational Development (GED) diplomas
Please contact the Louisiana Community and Technical College Assistance System (Adult Education).
Copies of High School Diplomas and Transcripts
Students can acquire a copy of their diploma and transcripts from their local school, school district or the Louisiana Department of Education.
Schools and school districts maintain permanent records and provide copies to the Louisiana Department of Education. Because reissued diplomas must be signed by the school district superintendent, school board president and high school principal, requests made to the Louisiana Department of Education must be sent to the local school district for signatures, which may prolong the processing of requests.
To expedite your request, please contact your local school district.
To request a copy of your diploma and transcript from the Louisiana Department of Education, please mail a completed and signed form with a payment of $5 per transcript and $20 per diploma (corporate checks, money orders and cashiers checks are accepted, personal checks and cash are not accepted) to:

The Department of Education is unable to verify that your application is received. To receive verification that your request has been received, please send your request via Certified Mail which will provide you with a receipt confirming delivery.
If a third-party individual or agency is requesting a copy of your transcript and diploma on your behalf, a signed statement authorizing release of the information, and a copy of driver's license or state issued identification, must be included with the completed form.
Fixing an Error on a Transcript
If there is an error on your transcript, please contact your local school district which is the only entity that can amend a school transcript.
